Adults recovering after hospitalization, rehabilitation stays, or post-surgical procedures including joint replacement.

Individuals experiencing balance impairment, fall risk, or neurologic conditions that affect safe mobility.

Weakness & Functional Decline

Those facing ongoing weakness, deconditioning, or functional decline requiring skilled professional therapy.

Preservation of Function

Patients who require skilled oversight to prevent decline and maintain their current level of independence.

Coverage
Medicare Part B & Direct Access

Next Stage Therapy provides outpatient physical therapy under Medicare Part B. Ohio law permits individuals to begin physical therapy without a physician referral through direct access.

Direct Access in Ohio

You can start physical therapy without waiting for a referral. Ohio law supports your right to access care directly.

Medicare Requirements
  • A licensed physical therapist establishes the Plan of Care
  • A physician must certify the Plan of Care
  • Services must meet medical necessity guidelines

You do not need to be homebound. If you do not yet have physician involvement, we can assist in coordinating certification to ensure coverage requirements are met. Deductibles and coinsurance may apply.

Each plan of care is documented, measurable, and physician-certified when required for Medicare coverage. Caregiver instruction is provided when appropriate.

When the Environment Matters

If broader housing planning or accessibility clarity is needed, those services are offered separately through Next Stage Living™, an educational housing planning service. Therapy and housing planning are distinct services — medical care remains independently delivered through Functional at Home, LLC.

FAQ
Frequently Asked Questions
Do I need a referral to start therapy?

Not necessarily. Ohio allows direct access to physical therapy. However, Medicare requires physician certification of the Plan of Care for coverage.

Do I need to be homebound?

No. This is outpatient therapy delivered in the home. Homebound status is not required.

Do I have to be improving to qualify?

Medicare may cover therapy when skilled oversight is required to improve or preserve function, provided medical necessity criteria are met.

How long can therapy continue?

As long as services remain medically necessary and require skilled care under physician certification.

Is Next Stage Living™ the same as therapy?

No. Next Stage Therapy provides medical physical therapy. Next Stage Living™ provides educational housing planning and home safety guidance. They are separate services.
